2nd chance registration on Saturday, September 20th from 8:30am to 10:00am.  Print and complete the forms below.  Please do not mail them - just bring them to the arena on Saturday and we will get you all of the information you need for the upcoming season.

The PDF document below contains all of the forms for registration, plus additional pages for informational purposes only.  Print the entire packet, complete all required forms, and mail it to the Faribault Hockey Assocation at the address below.

REMEMBER, UPON PRINTING THE ON-LINE REGISTRATION MATERIALS, YOU ALSO NEED TO GO TO THE VOLUNTEER SECTION OF THE WEBSITE (MENU ON THE LEFT) AND REGISTER IN THE VOLUNTEER SECTION AS WELL.  THIS WILL BE FOR YOU TO TRACK VOLUNTEER TIME FOR THE ASSOCIATION.

A limited number of scholarships are available to assist families with financial need.  Scholarships include additional volunteer requirements to help offset the financial assistance.   If you are looking for financial aid, please print and fill out the form below and return with your registration information.  These forms will be treated with the upmost confidentiality.  If you have any questions, please contact Jason Johannsen directly at 612-801-8917.
